Why Do Some Crowned Teeth Need Additional Internal Support?

There is a sentence that gets repeated in dental chairs and on the internet with great confidence and no accuracy: "the tooth is weak, so we'll put a post in to strengthen it."

A post does not strengthen a tooth. Decades of laboratory and clinical evidence point the other way — preparing a root to receive a post removes dentine from the one part of the tooth that is still intact, and a tooth with a post is, all else equal, structurally weaker than the same tooth without one.

So why are posts used at all? Because they solve a different problem entirely.

What a Post Actually Does

A post is a retention device. Its job is to hold a core in place.

When a tooth is very heavily broken down, there may not be enough tooth substance left above the gum to retain a filling material. The core — the block of composite or similar material built up to replace the missing tooth structure and shaped to receive the crown — has nothing to grip. A post extends into the root canal, is secured there, and gives the core something to be anchored to.

That is the whole function. Retention of the core, not reinforcement of the root.

Once you hold that distinction, the clinical rules follow logically. If enough tooth remains to retain a core without a post, a post adds risk and no benefit. If not enough remains, a post becomes necessary — and the additional risk is accepted because the alternative is no restoration at all.

The Core Build-Up Comes First

Most heavily restored teeth need a core. Far fewer need a post.

A core replaces missing tooth structure and creates a regular shape for the crown to fit over. Modern bonded composite core materials adhere to the remaining dentine and, provided there is a reasonable amount of tooth left, are entirely adequate on their own. The common rule of thumb is the amount of remaining coronal tooth structure. Where several walls of the tooth survive, a bonded core alone is usually sufficient. Where less than roughly half remains, retention becomes questionable and a post enters consideration.

Crucially, none of this substitutes for a ferrule — the band of sound tooth the crown must grip around its whole circumference. Core material is not tooth and does not count towards it. Why Molars Usually Need No Post

Back teeth have an advantage that front teeth do not: a pulp chamber.

The chamber is the hollow space in the body of the molar where the pulp sat before root canal treatment. It has walls, undercuts and depth, and a bonded core material filling that chamber gains substantial mechanical retention from it. Many molars therefore need no post at all, even when heavily broken down.

Front teeth have a much narrower chamber that offers little, which is why posts are used more often on anterior teeth despite those teeth carrying less vertical load.

There is a second consideration in molars. Their roots are frequently curved, sometimes markedly, and are narrower than they appear on a two-dimensional radiograph. Preparing them for posts carries a genuine risk of perforating the side of the root — a complication with a poor outlook. The combination of good chamber retention and elevated preparation risk means the default in molars is to avoid a post.

Fibre Post or Metal Post?

The two main options behave quite differently, and the difference that matters is the failure mode.

Fibre posts are made of glass or quartz fibres in a resin matrix. Their stiffness is close to that of dentine, which means that under load the post and the root flex together rather than the post concentrating stress at one point. They are placed at the same visit, bonded into the canal, and are tooth-coloured, which helps under translucent crowns on front teeth.

Cast metal posts and cores are made in the laboratory from an impression of the prepared canal, so they fit the canal shape precisely — an advantage in a very flared or oval canal where a prefabricated post would fit poorly. They are considerably stiffer than dentine.

That stiffness is the crux. When a tooth restored with a rigid metal post is overloaded, the stress concentrates at the tip of the post within the root, and the characteristic failure is a vertical root fracture. That outcome is not repairable; the tooth is lost, and because the fracture line allows bacteria to reach the surrounding bone, the site is frequently compromised for a future implant as well.

When a tooth restored with a fibre post is overloaded, the more common failure is debonding of the post and core as a unit. The tooth is still there, and in many cases it can be re-restored.

This is why fibre posts have become the more common choice in general practice. Failure rates are broadly comparable; the consequences of failure are not.

What a Post Does Not Fix

Several situations prompt requests for "extra support" that a post will not provide.

A tooth with reduced bone support is not helped by a post — the problem is below the tooth, not within it. A cracked tooth is not held together by a post, and a post may propagate the crack. A tooth with no ferrule is not rescued by a post; it is placed at higher risk by one. And a tooth that is generally "weak" because it is heavily filled but has adequate remaining structure gains nothing from a post but loses dentine to it.

The Long-Term View

A post and core, properly indicated and well executed, can serve for many years. The variables that determine whether it does are largely the same as for any restored tooth: whether there is a good ferrule, whether the root filling and the coronal seal are sound, whether the bite is controlled, and whether the tooth is protected from grinding.

Where grinding is present, a night guard is a meaningful part of the plan rather than an optional accessory, since a post-and-core tooth is among the least forgiving in the mouth under sustained lateral load. Our article on whether grinding can crack teeth covers the mechanism.

The final point is one worth raising at the planning stage. If a tooth needs a post because so little remains, that is itself information about the outlook. It does not mean the tooth should not be restored — often it should — but it belongs in the conversation about how many stages are involved and what happens if it does not work out.

Frequently Asked Questions

Does every root-treated tooth need a post?

No, and most do not. The decision depends on how much tooth structure remains above the gum, not on whether root canal treatment was carried out. Many root-treated molars are restored with a bonded core and a crown, with no post at all.

Is a post permanent once placed?

A fibre post can usually be removed if retreatment is needed, though not always easily. A cast metal post is considerably more difficult and carries some risk to the root. This is one of the factors in choosing between them where future retreatment seems plausible.

Will I feel the post?

No. It sits within the root canal of a tooth that has already had its nerve removed, so there is no sensation from it.

Does a post make the tooth stronger?

No. This is the central misconception. It retains the core. A prepared root has less dentine than an unprepared one, and less dentine means less strength.

Can a post come loose?

Yes, and with a fibre post this is the more common failure mode. It is generally recoverable — the post and core can often be replaced, provided the root is sound and a ferrule remains.

Why did my dentist say a post is too risky for my tooth?

Usually because the root is narrow, curved, short, or already has a prepared canal that would leave insufficient dentine around the post. In those situations the risk of perforation or fracture outweighs the benefit, and alternatives are considered instead.
